Medical Clearance Form From Your Doctor or Signed Medical Waiver

Federal law requires any person prior to being fit with hearing aids, must either obtain a Medical Clearance from a licensed physician or sign a Medical Waiver declining a hearing examination. You will be directed to provide an electronic signature for the Medical Waiver at checkout unless you provide a Medical Clearance form from your doctor first.

At your own expense, your doctor or hearing specialist may conduct a hearing test  and exam to insure there are no underlying medical conditions that need to be addressed. A Medical Clearance in the form of a simple "Script", similar to a doctors prescription form stating that you are medically cleared to wear hearing aids is also acceptable.

The physician may have a hearing professional conduct a hearing test for him/her. If this is the case, please ask for a copy of your hearing test results or Audiogram be provided to you.  Be prepared for the hearing professional to try to sell you their own hearing aids. Simply decline their invitation and request the audiogram copy. They are required by law to provide a copy at your request since its a part of your medical records.

Under federally mandated HIPAA regulations, health care providers must provide you with copies of certain documents when requested, including hearing aid clearance forms and hearing test or Audiogram results, so if they do not provide a copy you should inform them of your right to these documents.

Step Two - Provide A Copy Of A Recent Hearing Exam
   
Hearing test audiogramA complete hearing test is required by each patient, having been completed within six (6) months of the date of purchase of a hearing instrument, and which is mandated by the U.S. Food and Drug Administration. Our Customer Service Center can advise you about meeting this requirement. A complete hearing test quantifies hearing loss by determining values for Pure Tone Air Conduction, Bone Conduction, Speech Reception Threshold, and Speech Discrimination in an audiogram. Most audiograms cover the range of 250Hz to 8000Hz (8kHz), the area most important for clear understanding of speech. Audiograms display a plot of the threshold of hearing relative to a standardized curve that represents "normal" or "abnormal" hearing.

We strongly recommend that you see an Ear, Nose and Throat physician for a medical examination and hearing test to determine your need for a hearing instrument prior to purchase. You are required to complete the Medical History/Medical Waiver form prior to purchase if you have declined the right to a medical examination. It will also be necessary to sign and submit the Health Information Personal Privacy form (HIPAA) prior to your purchase.

Both forms can be completed on line by clicking on the links above. You may also print, fill in and mail or fax these forms, along with the results of your hearing test, to the DiY Audiology dept. (contact us for the correct email address or fax number) A member of our Audiology Department will call you within twenty-four (24) business hours of receipt to discuss the best solutions, and offer a recommendation for your particular case.

Once you have submitted the necessary paperwork and we have determined the level and nature of the hearing assistance you require, we will assist you with Step Three.

Step Four - Placing Your Order

Open Fit styles that do not require a custom made earmold can usually be shipped within 3-5 days of the receipt of your order. Custom hearing aids that do require an ear impression are built and shipped within two weeks of receiving your order and your ear impressions.

Payment is required at the time of your purchase, which is immediately following the hearing aid selection process.
